A doctor or medical professionals' referral is not needed for a patient to be referred for Hospice Care. Anyone can refer a patient for Hospice Care. Below is a Hospice Intake Referral form for Universal Home Health & Hospice Care.
Early referral is the key for Hospice Care. Once a person is diagnosed with a terminal illness of 6 months or less, it is recommended to contact a hospice agency as soon as possible. A hospice agency can provide education, assistance, and services to benefit the patient and patients' family.
